Covid19 - Implications of opening up to vaccinated travellers

 The losers in this pandemic have sought consolation in living with the virus and trying to live with the virus spreading in their communities. The primary consideration for the opening up after failing to control the virus is economic. The economy must be allowed to go on as many businesses depend on it to survive, to pay wages and rentals. 

So the new mantra is to allow vaccinated visitors in on the assumption that they would not die or be severely ill from the virus. It is NOT that the vaccinated visitors are safe from being infected or from not spreading the virus. Many official reports have confirmed that the vaccines are ineffective in preventing infection and that vaccinated people are in fact carriers of the virus and big spreaders of the virus. The risk of spreading the virus from opening up does not even consider the number of fake vaccination certificates that allowed dangerous carriers of the virus to spread the virus.

The biggest consequence of living with the virus is the acceptance of infection as a norm and the accompanying deaths, hoping that the numbers did not spiral out of control and remain at an acceptable level. The living with the virus policy also goes with the assumption that the vaccines would minimise the severity of the disease and deaths when it has proven so far that this is a lie, with the virus still spreading uncontrollable in many countries. Countries that are reporting lower infection are mostly faking their data by not testing or not reporting the truth on the ground. For those with underlying medical conditions, the vaccines are not going to help very much if infected.

The other hope or assumption is that now is the worse and things would be better, that there would be no deadlier variants to do more harm and damages. With the opening up and lowering the guards, doing away with restrictions, new variants would spread easily and faster and the crisis could only be bigger and deadlier.

Just pray hard that no more deadlier variants would appear as the opening up means minimal protection from new variants hitting the population. The guards are down, no firewalls to protect and keep the virus out. Just hope that all is well.

  2. Mdm Ho Ching was quick to help sell the Indian vaccines as replacement to Sinovac just days after WHO approval for emergency use.

    She had called out those who questioned Temasek Holding's heavy employment of South Asians PMEs as racist.

    Maybe she and her family should try the vaccines before recommending for our use.

  3. Covid-19 Singapore: Living With Covid-19 Is Banging On Too Many IF's

    There are too many "IF's" in this equation of hope against Hope against HOPE - one hope over another hope and again over another hope.

    Hope, hope, hope..... Banging on too many hopes. Full of hopes. Desperately optimistic!

    1. Hope that the vaccines do their work of preventing infections. And once we achieved 80% vaccinated, we will have herd immunity. Remember, both Pfizer and Moderna said that their vaccines' Efficacy are 95% and 90% respectively. Pfizer CEO and Moderna CEO must be very honest people. So, whatever they say must be the truth, the whole truth, nothing but the truth. So, help us God!

    This is banging on IF the vaccines worked wonders - once you took the jabs as prescribed, you will have the immunity against COVID-19. And once 80% of the population have been vaccinated, the whole country would be immuned because 80% of the people will not be infected. As such, we can open up the country to allow those silent and stealthy carriers of the Covid to come in and spread the disease as much as they can. We are immuned and we should not be afraid of getting infected and die unnecessarily anymore.

    2. Hope that IF the vaccines do not work, at least they can prevent severe illnesses and deaths. Remember again, Pfizer and Moderna said that their vaccines can at least reduce severe sickness and death by 86%? What these Big Pharmas' CEOs said must be true again, even IF the claim of 90-95% efficacy does not hold water. We, Singaporeans, are very trusting people!

    3. Hope that IF we allow those vaccinated people from other countries to come in, they are all immuned from Covid-19 because, just like us, they have taken the prescribed vaccines jabs already. So, they won't be carriers of Covid-19 viruses and they won't spread the disease amongst our population. Logical, right?

    4. Hope that IF we allow foreign very honest travellers and businessmen to come in via our designated VTL (Virus Travelling Lanes), they will be very honest and truthful and upright. They will not dare to deceive us by using fake certification for their vaccination status. They will also not bring along infected children below 12 years old, who do not need to be vaccinated. Foreigners are very good people, you know?

    5. Hope that IF we open our legs wide wide, for every Tommy, Dicky and Herring to come and screw us up good and proper, our healthcare system will not deteriorate and collapse. We are so, so, so confident that all our assumptions (IF's) are correct!

    Remember, for almost two years already, we have not only refused to expanded our healthcare system and staffing but instead we saw the exodus of thousands of our healthcare workers due to mass resignations. We just kept quiet and said nothing. Only now then we start to throw money at our healthcare workers in order to make them stay on. Only now then we start to mobilize the nursing students to help out. Remember, we don't anyhow depend on foresight, we are born and trained to depend on hindsight only. That's why we keep telling people "we will monitor closely", wait and see lah.

    And we know that fighting this Pandemic is like fighting a war, yet we are so Covid-Naive that we have not put our country on war footing. All our Ministers and MPs are still having a good and cosy, costly and comfortable life, safe and sound. We don't even bother to mobilize the Civil Defence Force and the Military to help out. We only mobilized a small part of the Police Force to show overwhelming force to a group of foreign workers who are not happy with how they have been treated in their dormitory.

    6. Last but not least, hope that after doing all that, our people will be safe. Infections will be reduced to the minimum (3,000 a day) and deaths will only occur to those who have underlying medical conditions.

  4. Never trust the Jinx's words. It is all pro-Indian propaganda. What medical expertise does she have to come out and say such stuff in support of the Indian vaccine? Sounds like she is now the spokesperson for vaccine diplomacy, on the side of the Indians, against China's Sinovac.

    The message she is discreetly giving about the Indian vaccine as 'replacement' for Sinovac is telling. Why is the Indian vaccine just a replacement for Sinovac alone and not Pfizer or Moderna? The message is clear and viciously vile, directed against Sinovac.

      Indian media is capable of churning fake news and India is notorious in presenting fake data, not saying they are faking on the vaccines.

    Don’t go gallivanting yet today!

    Yah, I know it’s Saturday, the favourite day every week for going out for makan and drinks with family and friends.

    True, the week on week growth ratio for our rolling 7-day new cases has dropped to 0.81 or a decline of 19% week on week.

    A 19% weekly decline means we would halve our new weekly numbers in 3-4 weeks.

    Why do medical and public health authorities track the cumulative 7 days worth of new cases on a rolling basis?

    Others track 14 days’ worth of new cases on a rolling basis.

    Rolling basis means every day, we just add up the new cases that we see in the past 7 or past 14 days.

    This is bcos folks are most infectious just before and just after symptoms show. And then the infectiousness declines.

    Last year, most covid cases were no longer infectious after day 7.

    This year, with Delta, with the higher starting viral load, 1000 times more than the now extinct variants, it may take longer for an unvaccinated case to become non infectious.

    And that is now clear.

    We now know that with Delta, vaccinated patients tend to recover faster, within 5-7 days, while unvaccinated patients take 10-15 days, double the time taken last when there were no vaccines available.

    Even for folks who need oxygen, the vaccinated seniors often only need about 3-5 days of oxygen support and they recover. Meanwhile, the unvaccinated seniors need more prolonged care with much higher risk of going into ICU.

    For both vaxxed and unvaxxed patients, having underlying covid risk conditions simply compounds their risk of complications.

    This infectious period is why healthcare authorities often track the rolling 7 days worth of new cases. It gives a sense of the likelihood of hidden active cases still in the community.

    And the week on week rise or decline of this rolling 7-day cumulative number of new cases above or below 1, gives us a sense what to expect in the weeks ahead.

    However, this week on week growth ratio tracks the flow, and hence is an early indicator of the flow into hospitals as some cases will worsen and become serious in 3-12 days after infection, rather becoming very sick immediately.

    So we should also track the stock of patients who need oxygen/ICU or die.

    While not all patients who die go into ICU, like the 99 year old who chose to die at home, many of those who die would have spent time in ICU and thus would occupy an ICU bed before death.

    For oxygen/ICU/deaths, we can also track the week on week growth ratio.

    This has flat lined at 0.99 or only a 1% decline, despite the 19% drop in the week on week new case numbers.

    This 1% decline in oxygen/ICU/death means it will take 50 weeks or almost a year to halve the number of serious Covid cases in hospitals.

    But this is a lag number, and a sticky slow moving number.

    Why is this lagging?

    The new cases today will translate into serious cases in 3-12 days later.

    Unvaccinated seniors can become oxygen cases quickly within a few days and then deteriorate into ICU cases all within a week, faster if they have underlying covid risks.

    Unvaccinated young people can be relatively well for a week or so, until their adaptive immune system kicks in. This happens between 7-12 days.

    Some young people may see an over reaction from their own immune system. This causes their immune system to attack not just the infected cells, but also the healthy cells and healthy organs.

    They spiral quickly into serious cases, including the need for ICU support or death.

  6. Hence, whenever we see a rise or fall in new cases, these translate into rise or fall in serious hospital cases in 3-12 days.

    So we have to watch that pressure flow into hospitals.

    But folks don’t typically go into and out of oxygen/ICU within a day.

    Oxygen/ICU/deaths are “sticky” numbers.

    People who died may have spent as long as a month struggling in the ICU. Others in ICU could be there for 2-3 weeks before recovering and transfer to high dependency wards.

    So the numbers of oxygen/ICU/deaths represent the stock rather than flow of serious cases.

    As mentioned earlier, unvaccinated patients are 9-10 times more likely to turn serious, and these serious cases often take up to 14-21 days before recovering or passing on.

    Vaccinated folks take half the time to recover, compared to unvaccinated folks.

    Hence, the unvaccinated form a disproportionate number of patients on oxygen, in ICU or dead.

    So we need to wait 2-3 weeks to see 2 things:

    1. Whether the week on week drop in 7-day new cases would maintain its downward trend;

    2. Whether the week on week ration of oxygen/ICU/death cases would similarly follow the sharp decline in new cases.
    Overall, we are in a good position.
    Our unvaccinated seniors have dropped to below 70,000.

    We are still having walk-ins, as well as home vaccinations.

    Call 1800 650 6060 to book a home vaccination team to come and vaccinate your bedbound elders at home for free.

    That home vaccination may not save the bedridden old folks from death from their old age or other underlying health risks, but will save them an uncomfortable death from Covid. It will reduce the guilt of family members who may feel they have brought the virus home to their parents or grandparents or other elderly relatives.

    Boosters are also going well, helping to reduce risk of serious illness or death even further.

    This has reduce the risk of oxygen/ICU/death on average to well below 2%.

    Vaccination reduced the risk of serious illness or death from 20% last year down to 2% in this Delta exit wave.

    This includes those on oxygen from about 15% to 1.7%, ICU from 3-5% to 0.1%, and death from 2-7% to 0.2% - these are numbers based on the worst case condition of each patient - those who progressed from oxygen to ICU are marked as ICU, those who needed only oxygen and never went to ICU are marked as oxygen.

    With boosters, we have seen those needing oxygen drop to 1% over the last 1 month - 0.8% needing oxygen and then recovering, another 0.2% needing oxygen but were in ICU as a precaution in case they need to be quickly intubated for high level oxygen support, but recovered without needing to be intubated as full fledged ICU case.

    The % of ICU and death remained stable at 0.1% and 0.2% respectively.

    This could be just an artefact of rounding, as 0.2% could range from 0.15% to 0.249%.

    But it also could be a hint that the deaths were replacement deaths.

    International studies that looked at excess deaths to estimate the “true” number of deaths from Covid find that Sg is among the minority of countries with negative excess deaths.

    Part of the reason for negative excess deaths, is that with all the covid precautions and restrictions, we also reduce the risk of death from other respiratory infections like flu, etc.

    Once upon a time, a king frog used to live in a well with his family and subjects. The king frog had some evil relatives who wanted to snatch the throne away from him. They kept causing problems in his reign. Once, they conspired with a minister in the king frog’s court and started a revolt against the king frog. The king frog managed to subdue the revolt but he was very angry. He wanted to take revenge on his evil relatives and unfaithful subjects. He thought of a plan. He came out of the well and made his way to a hole where he knew that a big, black snake used to live. He called out to the snake.

    The snake was surprised to hear a frog calling him and slithered out of the hole. The frog said to the snake, “Hello. I want to be your friend.”

    The serpent agreed. He followed the frog back to the well. On reaching there, the frog said, “Here are my evil relatives. You can eat all of them except my near and dear ones.”
    Thus, the serpent started living in the hole in the well’s wall. One by one, he started eating up the frogs that were pointed out by the king frog. In a few days’ time, the snake had eaten all the evil frogs and only the king frog and his family were left. The snake came up to the king frog and said, “I have eaten up all your evil relatives and rebels as you wanted. I have also eaten your unfaithful minister. Now, there is nothing left for me to eat except you and your family.” Saying this, the serpent eyed him hungrily.
    The king frog realized his mistake. He knew he was in great danger. He had made friends with his natural enemy for his selfish motives and to punish his enemies. Now he felt that his own death was near. But he managed to keep his cool and replied, “Do not worry. I will go to some other wells and ponds and persuade some frogs to come and live in this well. Then you can feed on them.”

    The serpent liked the idea. “Hurry up, then. I am hungry,” he said.
    So, with this pretext, the king frog and his wife came out of the well and ran for their lives, vowing never to return to that well again.

  18. A large part of the battle during this oandemic/endemic is fighting fake news, and unsubstantiated accounts. These are sometimes spread by politicians in the opposition. Among them are Brad Bowyer, Goh Meng Seng and Gilbert Goh.

    In a Facebook post, MOH wrote:

    MOH is aware of a Facebook post made by Gilbert Goh on 1 November 2021, in which he shared second hand accounts allegedly of:

    (i) a woman, under the age of 50, who had passed away from COVID-19 infection after being sent home from A&E twice; and

    (ii) a woman, aged 20, who was sent home from A&E despite experiencing shortness of breath. The female was then admitted to Tan Tock Seng Hospital, and diagnosed with lung infection.

    Based on our records, there are no cases that matched the descriptions in the post. These are serious allegations. We invite Gilbert Goh to come forward and provide us with the details of the patients and alleged cases, so that we can follow up accordingly.

    We wish to remind members of public that transparency and accuracy of information undermines public confidence in a pandemic crisis.

    Hence, when sharing personal experiences by individuals, we need to ensure that they are substantiated, so that the public does not end up consuming rumours and untruths.

  19. Singapore, Malaysia to launch vaccinated travel lane between Changi Airport and KLIA

    Singapore and Malaysia will launch a vaccinated travel lane (VTL) between Changi Airport and Kuala Lumpur International Airport from Nov 29.

    This was announced in a joint statement by Prime Minister Lee Hsien Loong and his Malaysian counterpart Ismail Sabri Yaakob after both leaders spoke on the phone on Monday (Nov 8).

    "The Prime Ministers recognised that COVID-19 has disrupted people to people connections between the two countries, and separated families for many months," read the statement.

    "Given the significant progress that both countries have made in vaccinating their respective populations and managing the COVID-19 pandemic, the Prime Ministers agreed that it is timely to progressively resume cross-border travel between both countries, in a safe manner."

    Under the VTL, fully vaccinated travellers will be able to travel between Singapore and Malaysia, and be subjected to COVID-19 tests in lieu of serving quarantine or stay-home notice.

    Under the scheme, travellers must have remained in one or more of the VTL countries in the last 14 days before departure to Singapore.

    They must take two polymerase chain reaction (PCR) tests – before departure and on arrival at Changi Airport – and enter Singapore via designated flights.

    The PMO added that both leaders also looked forward to restoring travel across the land links "between both countries in the near future".

    Unvaccinated children aged 12 and below are allowed to travel under the scheme – except to South Korea – as long as they are accompanied by a traveller who meets the requirements.

    Singapore currently has VTLs with Australia, Brunei, Canada, Denmark, Germany, France, Italy, the Netherlands, Spain, Switzerland, the United Kingdom and the United States.

    The scheme will also be extended to South Korea from Nov 15.

  21. Only last night then we are told that there have been at least 8,000 pediatric cases (children infect with COVID-19) in Singapore!

    We are hearing this for the very first time!

    And we have extremely highly paid top civil service and administrative service and top-notch very efficient and very competent and very highly educated scholars and ex-army generals in the government! Just imagine that!

    Things just don't jell! This cannot be! This cannot happen in the most "kilat" country in the world, man!

    I am completely baffled and flabbergasted!

    1. COVID-19 Singapore: Compelling People To Get Vaccinated Is Officially Declaring That VACCINATION IS COMPULSORY

      SINGAPORE: From Dec 8, all COVID-19 patients who are unvaccinated "by choice" will have to pay their own medical bills if they are admitted to hospitals or COVID-19 treatment facilities, the Ministry of Health (MOH) said on Monday (Nov 8).

      "Currently, unvaccinated persons make up a sizeable majority of those who require intensive inpatient care, and disproportionately contribute to the strain on our healthcare resources," said MOH.

      The new rule will apply to COVID-19 patients who are eligible for vaccination but choose not to do so.

      Those who are partially vaccinated will have their medical bills paid for by the Government until Dec 31 to allow them time to be fully vaccinated, MOH said.

      COVID-19 patients who are unvaccinated by choice may still tap on regular healthcare financing arrangements to pay for their bills where applicable.

      Singaporeans and permanent residents may access regular government subsidies and MediShield Life or the Integrated Shield Plan where applicable.

      Long-term pass holders may use their usual financing arrangements, such as private insurance.

      People who are ineligible for vaccination, including children under 12 years old or medically ineligible patients, will still have their COVID-19 medical bills fully paid for by the Government, the Health Minister added.

      From Jan 1 next year, only Singaporeans, permanent residents and long-term pass holders who are fully vaccinated and have not recently travelled to other countries will have their COVID-19 medical bills fully paid for by the Government.

      MOH said the current arrangement of footing the full COVID-19 medical bills for all Singaporeans, permanent residents and long-term pass holders was to "avoid financial considerations adding to public uncertainty and concern when COVID-19 was an emergent and unfamiliar disease".

      "For the majority who are vaccinated, this special approach for COVID-19 bills will continue until the COVID-19 situation is more stable," the ministry added.

      Speaking at a multi-ministry task force press conference, Health Minister Ong Ye Kung said hospitals would prefer not to have to bill COVID-19 patients who are unvaccinated by choice.

      "But we have to send this important signal to urge everyone to get vaccinated if you are eligible,"
      he added.

  25. This step was more or less anticipated. When this still could not force those unvaccinated to comply, the next step they will take may be to declare that those not vaccinated and with serious complications cannot be treated in Government Hospitals. Does the name Hell Minister mean anything?

      Taking a different mRNA vaccine as a booster will lead to an equally effective outcome as sticking to the same vaccine, said Singapore's director of medical services Kenneth Mak.

      Speaking at a press conference on Monday (Nov 8), Associate Professor Mak cited from his personal experience to assure the public. He had taken the Pfizer BioNTech/Comirnaty vaccine for his first two doses, but a Moderna jab for the booster.

      "It's okay to cross over," he said, noting that his choice was partly due to convenience as the vaccination centre he had gone to offered Moderna vaccines.

      He added: "This is perfectly safe and provides adequate continued protection against Covid-19. If you are eligible... get your booster vaccinations within the range of vaccines available that the expert committee has recommended."

      He gave the example of Israel, which predominantly uses Pfizer-BioNTech vaccines for booster shots, but is now also using others including the Moderna vaccine. The country has shown "excellent results" in bringing the Covid-19 situation under control, he said.

      Health Minister Ong Ye Kung also said he had taken Pfizer-BioNTech for his first two doses, and Moderna for his booster shot.

    Singapore is among the countries with negative excess deaths during the pandemic

    International analysts compare deaths during the pandemic year, to the death rates before pandemic.

    This is a way to assess the “extra” deaths due to the Covid pandemic.

    A minority of countries saw less deaths compared to pre-Covid years.

    This is partly the result of the infection control measures like masking, washing hands, safe distancing. These reduce the rate of infection from flu and other respiratory infections.

    And so we see less deaths from flu and such, despite the deaths from covid.

  27. Why need pressure and not just make it compulsory. When less important things can be compelled, why is this step so difficult to take if it solves the problem.

    Persuasion tinged with discrimination, deprivation and veiled threats is force by another name. Just call a spade a spade and be done with it.

  29. A drug addict once told me how wonderful the drugs he took. He claimed that it was the best thing in his life, no side effects. Damn shiok!

    A few years later met him again in the drug rehab centre. He cried for being so silly to take the drugs for short term enjoyment and now suffering long term effects.

    He regretted for not taking my advice, but too late. Once consumed, the drug would be inside the body for life. There is no escape.
