Govt hospital as profit making business

I wrote about this subject many years back, that with the increasing medical fees and profits in the health business many govt doctors and specialists would be attracted to leave for the private sector to make their millions. The smell of money is intoxicating. And there is no need to utter silly things like big sacrifices. This is only human nature. Everyone in any profession would be attracted and want to make as much money as they could. Staying and working in the govt hospitals would no longer be a good choice except for those that value their contribution to the bigger good of society and patients more than money making.
 

As such it would be expedient and practical for some sort of arrangement or combination to allow govt hospital doctors and specialists to enjoy some monetary perks by offering their expertise to private patients at market rate. Govt hospitals could set aside a certain percentage of hospital beds and doctors/specialists for this purpose to take a share of the medical tourism business.
 

A main benefit for such an arrangement is to retain more doctors/specialists in govt hospitals as otherwise they would leave for the more rewarding private sector. Another benefit is that the profits generated could be used to lower the cost of subsidised patients in govt hospitals. In this way, both the doctors/specialists and subsidized patients would be winners, and lesser staff turnover problems for govt hospitals. The govt doctors and specialists would be recognized and rewarded to some extent for their skills and expertise without having to envy their rich peers in the private sectors.
 

The caveat is that there must be enough capacity in govt hospitals to take care of full paying private patients and the general public on subsidized rates. This is basic in managing skills. Carefully tweaked this could be a win win solution for all parties.
 

I just read in theindependent that this scheme was actually in practice for a number of years but would be stopped because of the short supply of beds in govt hospitals. This is from theindependent.sg, ‘After nearly a decade of not banning such practices, MOH has now told hospitals that they are no longer allowed to “actively market themselves to foreign patients” since the priority of public healthcare institutions must be to serve Singaporeans’ healthcare needs.
 

MOH’s decision comes after the public hospital bed crunch across Singapore, in recent years.
In one notable case in 2014, CGH pitched tents on its hospital grounds to accommodate patients after reaching 100 per cent bed occupancy and despite renting wards from private hospitals to hold patients….’

With the termination of this arrangement, I think it would become a lose lose situation for all. And the cause of it, not enough hospital beds. Why? Don’t they plan ahead in view of the greater demands for hospital beds and services? I can think of the bicycle sharing schemes, a good scheme but short in planning. They never cater for more bicycle parking lots and regulations to stop littering of bicycles everywhere.
 

When there is high demand for such medical services, why don’t they build more hospitals and provide more beds? Not so simple? KNN, super talents with super out of this world pay cannot find a better solution than just simply stopping a good scheme that benefits everyone, including govt revenue, cannot plan ahead? This business is high value added, good profits, anytime better than promoting more kopitiams and hawker centres for low and semi skill labour and low returns that befit third world countries aspirations, or building more shopping centres.
 

Medical tourism is good business, highly reputable and highly desirable business requiring high skilled professionals, and a little planning and foresight could avoid this simple logistic problem of demand and supply.

This showed that the little red dot Garmen has short changed its citizens for many years already. What's wrong with collecting more monies is their modus operandi. Even Garmen depts r looking for ways to make more monies even in the RC centres r doing that, Aunty Lucy also wanna buy Copidiam for market manipulation & profiteering thor dey claim to be social enterpize. Is this kind of a indirect corruption of some kind? Probably the Garmen worried the hospice capacity issue wud backfire them in next erection. After erection, when dey win big they wud they back the whole chicken plus interest.

Lets face it Redbean, if you were a young doctor, or if one of your children graduates as a doctor, you or your child would also be attracted to the big money. In Singapore, a place where money talks the loudest, personal sacrifices for the great good would be at the bottom of the list of priorities. In this little red dot, it is every person for himself or herself. Starting from the top, do you think all your government top position holders are there for the commoners? They are there to fill their own coffers and making sure that their children are in positions to take up the positions they will vacate. Here in Singapore, the most prominent belief is every person for himself or herself. Brush aside all who stand in your way. Get to the top and try to stay at the top, just as the example shown by your government the PAP.

When the country has a person who does not take responsibility for unnecessary deaths of patients caused by negligence of the government hospitals, for massive leakage of patients' personal confidential information and other ruthless scheming such as compulsory life insurance premiums collection from CPF members, the people should know you have a very serious problem in the countrt: The Problem of Leadership!

You DON'T have leaders. You have managers and administrators. Worst still, most of these are greedy, sycophantic and afraid to face the music when something goes wrong under their watch!

This problem does not confine to only one ministry. In fact, it is across the board, the worst ones are ... You can name it yourself.

Rb, Knn lar. From personal experience, even if u get admitted via A&E in the morning, the earliest they can get a bed for u will be late afternoon. Plus hospital now have a rule to kick u out after x no. Of days after surgery even if u are prepared to pay cause they just don't have enough bed. So as a patient u either stay in A&E ward if u are lucky or along the corridor. Many years back I went for surgery so I reported like 8 am in the morning. They did my surgery closed to 5 pm so I was lying on the bed at corridor at the surgical
ward for 8 hours!😰 So u tell me lar Knn or not.

I worked in govt hospitals until 2017...

Doctors in govt hospitals now all earn enough salaries/bonuses to buy landed properties after 5 years in the job.

The only difference is whether terrace house in Choa Chu Kang or GCB in Bt Timah --- depends on individual doctor's ambition, ability, politicking powers & luck.

As for medical tourism --- that is so 2000s. PAP already tried to promote it & spent millions advertising to other countries, especially in Middle East.

But it died by 2010. Rich people rather go to US where the cutting edge medical R&D, drugs, procedures, robotics, equipment are being invented & rolled out. Those not so rich will rather go to Thailand or even M'sia private hospitals.

S'pore healthcare is neither here nor there. If you bother to check the admissions into the private hospitals (e.g. Raffles, Mt Elizabeth etc) you will see that it have been going down over the years.

When they brought in third world doctors and charged their patients first world fees, what to expect? It is like expecting salaries of our workers to go up and up without productivity and quality go up. So must continue to bring in third world workers. They think all the rich patients are as daft as Sinkies.

Singapore is pricing itself out from shopping, medical tourism and everything. Everyone wants to be paid millions but not worth the price paid. This cannot go on for long when they have alternatives or substitution/competition. Only in political office they can continue to demand to be paid in the millions. No competition.

Let the doctors make their money in the private sector when there are lots of rich patients willing to pay for their service. Govt doctors should not be deprived of this lucrative source of income if you want to keep them in govt hospitals.

Work out a scheme that is win win for them and subsidised patients. Allowing govt hospitals to make some revenue from medical tourism is a good thing provided they don't neglect to take care of subsidised patients. There can be a balance.

You have to be dreaming mate. 🙄 I pay 1.5% Medicare TAX, only because I shell out another bunch of money for private healthcare. Otherwise the motherfucking Federal Govt will wallop me with another 1.5%! No such thing as FREE lah! Someone, (usually you and I) always ends up paying thru TAX which is mandatory. (no escape)

Last week I just had lunch with a group of Singapore investors who bought a couple of development plots in the Perth metro area for dirt cheap prices. The Aussie dollar is down too, so those guys really did ok. For one particular project they got 100% financing from an Aussie bank. i.e. 100% OPM. ZERO dollars of their own money. 👍🏻

I’ve said it before: I CHAMPION the Singapore healthcare system. I think the PAP got the mix between public and private just right in the sweet spot. I’ve been a patient, and have only praise for the system and the great people who work there.

IMO, you guys are fucking nuts. The govt doesn’t always get things “right”. However give credit where credit is due: the PAP have really done an excellent job with healthcare. And the best part: IT IS CAPITAL EFFICIENT, plus it has SUPERIOR outcomes (when compared with other 1st World cuntries)

I knew of 2 advanced Singaporean cancer patients basically told by their onco at a public hospital...no more treatments, hope u still around in 3 mths time. A kind fellow sister arranged for them to see her private onco n pay for their treatments. Now the 2 patients are better after 3 mths. The grateful patients want to reduce the kind sponsor financial burden by wanting to request the public hospital onco to do the scans but not approved. Indeed quite a heartless system...."you died your own business" by our very rich, voted by majority Sinkies Garhmen. How sad n disappointing.
